Editorial


Wishing all our KOA members warm greetings and a very happy new year from the JKOA editorial team. We are delighted to bring you the August-September 2022 issue of KOA Journal. We express a sincere thanks to all our members for their valuable support in submitting scientific articles which made this current issue possible.
This issue brings a refreshing review on the topic which remains a thorn pricking on all our members. Complex Regional Pain Syndrome continues to provoke our imagination and this review comes out with relevant new information on diagnosis and evidence for management. We have a succinct description of a surgical technique on a modified posterior approach to the hip. There are three very interesting case reports which mentions of unique challenges we face in dealing with not so uncommon problems in our practice.
The Editorial team wishes to receive many more articles on the good scientific work being done by our members.

Scientific research has always been an arduous task, involving patient and meticulous search on the library shelves and internet sources for past literature, doing thought-provoking experiments, documenting their productive results, writing and publishing reports. All this demands efficient use of time with a well-planned program, which includes close monitoring and supervision of projects, writing proposals, and scientific reports, while avoiding errors, quoting the correct citations, connecting networks, and searching for relevant journals for publication. How do we all plan for this challenging task?
Mercifully there are numerous online tools for the novice to help them organize their documents, search and cite literature, communicate and connect with professional contemporaries, and even more. An array of options are available, although the right tools yielding the best results may often be difficult to find. Here are a few options which I feel may be useful for our members.
• Project management tools like Trello and GanttPRO help manage project progress utilizing hierarchical listing that includes scheduling activities, formulating action plans, setting deadlines, resource allocation, establishing completion deadlines, and viewing progress.
• Innovative grammar check tools are now a ‘must have’ aid, detecting spelling, sentence and punctuation errors and correcting grammatical flaws. Grammarly and Trinka AI are two popular grammar checker tools available for researchers, which enhance the quality of language in the manuscript.
• EndNote and Mendeley have used reference management tools for correct citation and linking references in scientific articles.
• The bane of plagiarism can now be managed with numerous plagiarism checker tools such as Turnitin and Duplichecker. There are currently many journal finder tools, like Elsevier journal finder, which help send our article to the right and appropriate journal and save our precious time.
• Progress in research is vitally dependent on sharing individual scientific work for which exchange of ideas and connection between researchers. A great help encouraging collaborative effort is to connect through scientific, social networks sites like ResearchGate. These forums promote and publicize the research done in the scientific community and establish links between teams across continents. Many of these sites have a free trial version, which researchers, in the beginning, can use.

We congratulate our contributors to this edition and hope for more contributions from all our members as we finally move away from this pandemic. Have a great academic feast at KOACON 2022.

It is heartening to see the decline of the current pandemic which has taken a significant toll of our work, health and other valuable resources as we limp back to normalcy.
The editorial team of the JKOA has been working tirelessly to bring new information in Orthopaedics and with this issue, we continue the endeavour to showcase the good work done by our members. It has been a privilege for the editorial team to serve KOA and with immense pride and responsibility as we present this new issue for the members. We congratulate the authors for their innovative scientific work presented in this issue.
Scientific research and publication entirely stands on the honesty and integrity of the authors and the editorial teams of journals worldwide invariably are dependent on the sense of trust in their presumed ethical study. It is a matter of great concern to see an increasing frequency of misconduct in scientific publishing which include various forms of plagiarism, Salami slicing, manipulation of authorship and bias based on presence of potential conflict of interest.
Duplicate publication is one such example where the authors submit their papers to multiple journal at the same time. The authors withdraw their manuscript as soon it is accepted in another journal. This appalling act amounts to disrespect for rules, guidelines and above all the ethics of publication. It results in loss of precious time and resources for the journal editors, reviewers associated with issues related to copyright. The authors should be aware that the journals frequently communicate with each on ethical issues and a failed response from the authors or their institution could jeopardise their paper. Our journal follows the COPE guidelines diligently which clearly mentions the problems and solutions related to duplicate submissions. Many of the journals state in their information for authors including that of ours, that dual submission is unethical and subject to automatic rejection.
In recent times, academic competition have become more intense in our institutions which has lead to the adage of ‘publish or perish’. It is regrettable to see the rise of predatory journals which have made significant contribution to the rise of such academic misconduct. Such journals ignore the vital steps of peer review and end up as low-quality articles which unfortunately offers unfair competition for genuine hard working authors for academic positions. A more sinister consequence would be risks for our patients if scientific results of publications lacks credibility. It is extremely important for all the authors to be aware of the current ethical guidelines for publication.
Professional integrity in clinical work and research is absolutely essential for research to offer the best possible care for our patients. It is also very vital that we guide our younger generation to practise in both these spheres with the highest ethical standards.

Dear All,

Nothing else can be better evidenced by the above statement than the current situation we are in. The pandemic made us realise the daunting challenges in public health, tribulation in maintaining the normal rhythm of life and immense opportunities to grow and rediscover our potential and strength.

For the orthopaedic surgeon, in particular, assuming the role of front line warriors along with other health care teams, adapting novel ways of performing surgeries in these trying times, embracing online learning and teaching and now, finally planning offline academic events, life is surely coming towards a full circle.

The catastrophe of the pandemic showed surgeons how innovative methods of telemedicine, webinars, virtual conferences and meetings can still keep us connected and cause no dent in our academic pursuits. Yes, there is the dark side of this pandemic which resulted in many of our colleagues losing lives, livelihood and going through one of the turbulent periods of our lives. We bow to their sacrifices and stand in support of their dear ones.

The publishing team has continued to work to not disappoint the authors who have kept faith in us and hence, this issue is being published in its usual schedule. I thank the entire team for this, along with our esteemed reviewers for the journal, KOA members and the office bearers for their constant support.

Dear KOA members and Readers,
We are delighted to present the second issue for the current year 2019 with some thought provoking and interesting scientific work from our authors. We take this opportunity to thank them for their support in submitting scientific articles which has made this issue possible.

The KOA journal is committed towards integrity and good clinical practice. In this issue, our authors have come up with some interesting observations which challenges conventional presumptions about some of the commonly done procedures in our clinical practice. It is commendable to observe new findings from the authors on ubiquitous issues like the use of drains in surgery, role of antibiotics in open fractures, benefits of exercises in plantar fasciitis and the changing trend towards use of newer antibiotic delivery systems in bone infections. These recommendations underscore a pressing need to develop new guidelines and norms based on evidence based medicine. This will pave a long way in helping our Orthopaedic fraternity in offering a comprehensive and meaningful treatment for our patients who now seek more transparency in information and therapeutic options due to their easy access to internet and social media.

Presence of uniform clinical guidelines would be an important step towards gaining confidence of our patients and sealing the element of trust in the doctor- patient relationship. As we move towards a society where government sponsored medical insurance like the Ayushman Bharat will have a significant influence on the cost of medical treatment, a lack of accountability in what we convey will only add on to our vulnerabilities. This has been shown with alarming regularity in social media as the general public keeps reading about the increase in breach of trust with the medical profession. Use of such clinical guidelines would improve the respect and integrity of our profession and protect us from future medico legal liabilities. It is heartening to see the Kerala government has taken an initiative to come up with establishing clinical guidelines for primary health care at level one centres. We must wholeheartedly support such initiatives and take it up in our association too. One additional way would be to foster scientific temper in our society by encouraging more academic work, meetings and educational programs. More importantly, let us read more of literature in our field which should inspire us to produce genuine original work as has been done by many of the member authors in the current and past issues.

Editorial team hopes to receive further support with original scientific work from all our members and readers for future publications as we move towards an era where authentic data reigns supreme.
